Time Explored Looking Back at 6 Years as a Physician Assistant

It has been sixyears since I began working as a Physician Assistant. I still cant believe it. When the NCCPA sent me my first PANRE reminder letterI figured they must have miscalculated. But, it's true,I have been working sixyears in the field, I have some newgray hairs to show for it. On this date, we as PAs have to recertify, it is a chance to see how much we know or find out how much we have forgotten. In reality there is quite a bit of both, probably more of the latter. I am looking forward to the future, contemplating the past, thinkinga lot about my next six years. Where will I be the next time I sit to take my PANRE? Is this where I should be now? I love my job andconsider myself privileged in this matter? How many people can go to work each day and look forward to being there? I am surrounded by wonderful supportive people, doing something I truly believe in. I am grateful for my work, grateful for anorganization that considers us more than cogs in a wheel. We are given freedom to be more than just pushers of patients and providers of numbers. We have autonomy in harmony with teamwork. Aroma Compounds and Their Odors

An odor or odour is a volatile chemical compound that humans and other animals perceive via the sense of smell or olfaction. Odors are also known as aromas or fragrances and (if they are unpleasant) as reeks, stenches, and stinks. The type of molecule that produces an odor is called an aroma compound or an odorant. These compounds are small, with molecular weights less than 300 Daltons, and are readily dispersed in the air due to their high vapor pressure. The sense of smell can detect odors are extremely low concentrations. How Odor Works Organisms that have a sense of smell detect molecules by special sensory neurons called olfactory receptor (OR) cells. In humans, these cells are clustered at the back of the nasal cavity. Each sensory neuron has cilia that extend into the air. On the cilia, there are receptor proteins that bind to aroma compounds. When binding occurs, the chemical stimulus initiates  an electric signal in the neuron, which transmits the information to the olfactory nerve, which carries the signal to the olfactory bulb in the brain. The olfactory bulb is part of the limbic system, which is also associated with emotions. A person may recognize an odor and relate it to an emotional experience, yet might be unable to identify the specific components of a scent. This is because the brain doesnt interpret single compounds or their relative concentrations, but the mix of compounds as a whole. Researchers estimate humans can distinguish between 10,000 and one trillion different odors. There is a threshold limit for odor detection. A certain number of molecules need to bind olfactory receptors to stimulate a signal. A single aroma compound may be capable of binding to any of several different receptors. The transmembrane receptor proteins are metalloproteins, probably involving copper, zinc, and perhaps manganese ions. Aromatic Versus Aroma In organic chemistry, aromatic compounds are those that consist of a planar ring-shaped or cyclic molecule. Most resemble benzene in structure. While many aromatic compounds do have an aroma, the word aromatic refers to a specific class of organic compounds in chemistry, not to molecules with scents. The human nose is so sensitive to thioacetone that  it can be smelled within seconds if a container of it is opened hundreds of meters away. The sense of smell filters out constant odors, so a person becomes unaware of them after continuous exposure. However, hydrogen sulfide deadens the sense of smell. Initially, it produces a strong rotten egg smell, but binding of the molecule to odor receptors prevents them from receiving additional signals. In the case of this particular chemical, the loss of sensation can be deadly, as it is extremely toxic. Aroma Compound Uses Odorants are used to make perfumes, to add odor to toxic, odorless compounds (e.g., natural gas), to enhance the flavor of food, and to mask undesirable scents. From an evolutionary standpoint, a scent is involved in mate selection, identifying safe/unsafe food, and forming memories. According to Yamazaki et al., mammals preferentially select mates with a different major histocompatibility complex (MHC) from their own. MHC can be detected via scent. Studies in humans support this connection, noting its also affected by the use of oral contraceptives. Aroma Compound Safety Whether an odorant occurs naturally or is produced synthetically, it may be unsafe, especially in high concentrations. Many fragrances are potent allergens. The chemical composition of fragrances is not regulated the same from one country to another. In the United States, fragrances in use before the Toxic Substances Control Act of 1976 were grandfathered in for use in products. This is not the first time that this company has been faced with adversity. The first time was in the 1960’s when shopping went from downtown locations to more uptown locations in malls. The company transitioned to mall locations to cope with the change. This time the change did not come easy to the company. In fact this change has cost the company millions. This time JC Penney’s was faced with a challenge that they wanted to change. They wanted to transition the public’s perception of them. They no longer wanted to be viewed as an old fashioned department store. The company no longer wanted sales or clearance racks. They wanted to change the whole retail climate. They called it fair and square pricing (Baskin, 2013). This came off a lot like Wal-Mart’s always low prices campaign. This sounds like a great idea to me. However, it failed for many reasons. The main reason because it was confusing to consumers. While the other main reason being poor marketing. Many people sat in anticipation of this new campaign by JCPenney’s. There were just as many supporters in the beginning as well. When I heard of this I thought of an upscale Wal-Mart. Low prices I do not have to shop for sales anymore because these should be low prices every day. However, very shortly after this I found myself not shopping there at all. Consumers want a deal, and they do not feel that deal when they shop there anymore. It is the thrill of the hunt for consumers. Not only that but the sales ad and clearance racks used to change. They are no longer changing prices so there is no need to go daily, weekly, or even monthly. Customers may check there as a way to show case, but they are not buying. Without the sales and without the sale advertisements the company is not bringing in nearly the amount of people that were coming into the store to score the best deal. Next the advertisements they are sending out are worded poorly. They are no longer doing sales but they do mail out what they called month long value. Customers did not understand the wording of it. It was never broken down for them. Ideally they had sales, but they were not called the standard name. Therefore, customers missed out on them and they were not bringing in the clientele like a â€Å"sale† probably would have. They were not able to embrace JC Penney’s new tactic. Another problem with this campaign is that the average consumer does not know what the clothing costs. Therefore they think it should be or could be marked down. They have no idea if they were getting a good deal or not. Again the thrill of the hunt is gone, and still makes the customers confused. It was confusing to customers and that means there is a problem in marketing. When a place makes changes that could potentially be confusing marketing is the key. However, there advertisements were so irreverent that they made even less sense to begin with. They came up with a campaign after their numbers dropped called â€Å"do the math. † It was supposed to show how much easier it is to just get a low price in the beginning rather than use a coupon. This action failed for the company. The CEO Ron Johnson came out and reported later that â€Å"it was confusing† to some of their consumers (Baskin, 2013). It’s no wonder that they lost customers. They did not target other competitors about their prices just what the company was trying to do. Last but not least they attempted to open little stores inside their stores. It was a Martha Stewart collection like IKEA. Even that failed because Martha Stewart was not able to put her name on it, because she was still in litigation over her brand. So, it was still branded as JC Penney’s. Not that the name would have made much difference, but it was not thoroughly hought out within the company. Also, this is not a new tactic stores have been doing this for years. The renovation of the stores to add in this small store was costly. It has cost the company millions of dollars. It has depleted their cash, and has also caused their credit rating to drop (Baskin, 2013). This was a costly decision to make when sale s were already down. Here is the largest problem that they had they wanted become a high end store in a low end economy. If I were the CEO of JC Penney I would make quite a few changes. My first change would have been to go back to traditional wording for now. These are the words that customers are the most familiar with. I understand that some companies like to do all their changes at once because it is cheaper. However, when you are changing familiar terms it is wise to do it slowly. Or at least explain it as thoroughly as possible. Change is needed as a society, but no one likes change, because of this I feel that they should be done slowly and over a period of time. I would also have changed the price tags on their merchandise. In order to make someone feel like they are getting a deal I would change how they were priced. I would put a suggested retail price and then put â€Å"our† price on the label. This would appear to customers that they were getting a deal. Sales are because the prices are higher than what they need to be. The advertising is all about â€Å"trickery† to pull people in. In reality they were still doing sales but they were not called sales, and people did not buy into it. By changing the price tags on the items the customers are still getting the thrill of the buy. They can see what their item is going for at their competitor’s location, and impulse buy. This helps eliminate â€Å"showroom† shopping. Or leaving to check their prices somewhere else. If it is a matter of a few dollars they will not go back to purchase. However, if they can see the deal they will buy. Instead of focusing on expanding a business inside of an already expanding business I would have spent the money elsewhere. Imagine if they could have established new rules for buyers. Gone back to JC Penney’s original roots and prove their claims. It could have created new financing and lay-away policies that communicated value, and used social media to create meaningful communities of consumers who wanted to track and participate in conversations about prices. Employees could have been recruited and trained to offer a fundamentally new customer experience based on integrity. They could have changed the way Americans shop and feel they should stop. I would not have wasted money on an advertisement that was bewildering. I would have spent money marketing on calling competitors out on their prices. Sharing the news on how Penney’s was changing. How they were forward looking. Instead of making confusing ads with no sales just to avoid the word sale I wouldn’t have tried to stay away from it. Since they were still doing sales but not doing sales on certain items. Limiting the sales options were not the problem the problem was using unfamiliar wording. Measuring some of these techniques could be hard to do. Going back to traditional wording would be one way that is hard to track. However, I believe it would go hand in hand with how you would track the new price tags. That would be sales. With these new changes and advertisements I would think that sales would increase. I would not look at the actual accounting book but do a twelve month comparison on the sales on each individual store. This is time consuming and costly but I think it is the only way to see how each store is doing in comparison to how they were doing the previous month and year. During high sale times I would make sure I would have as much staff as possible on the floor to assist our customers. Maybe they do not need help but a casual conversation can lead into why they came into this department store and not the one across the way. Along with this I would like to institute team meetings once a week where department heads meet with their front line employees on all shifts. To find out their ideas and where they are hearing concerns are. Then I would have them write them up and do a teleconference with each store head to hear these ideas, questions, or concerns. I feel this is an open door policy. I would also include suggestion boxes not only in the store, but in the break room for employees so they could bring these up anonymously if they felt the need to. Also, I would work on getting the contact information to employees for everyone in charge. Change can happen and many great ideas come from the front line, because they see and do it every day. However, their voices are not often heard. To measure the effectiveness of advertising I would do a few things. I would add a survey at the end of their receipt to figure out what they thought about the advertisement. I would also add a quick questionnaire in the store that the customer could fill out. I would also make it known that there is a number they can call at any time with questions. I would make it so that they could be heard with questions and concerns. Before I launched a campaign I would have a test market so that we could see what people could recall from the add, as well as find out if there was any confusion on what may have been advertised. The sales would play a large part also in whether it was an effective campaign. A company that has been operating for 100 years is struggling. This way, the story would appeal to both adults and children. The production of Snow White cost about i 1. 75million but brought out a grand total of i 4. 2million. It was a great risk for Disney to take and if it hadnt been successful would have cost him his entire studio. Disney managed to capture the attention of his folly blind audience, as they were called, by producing thousands of individual sketches which gave the tiniest bits of detail. The death scene as well brought out a lot of emotions in the audience and tears in peoples eyes. Dream Works made Shrek in 2001, over two generations later. It was a direct parody of Snow White and other fairy tales like Sleeping Beauty (when Fiona wishes to be kissed by a handsome prince) and Peter Pan (when Donkey flies and a voice of Peter Pan is heard). It came from a childrens illustrated book by William Steig. Unlike Snow White, the characters in Shrek do no sing in a magical style but instead carefully chosen sound tracks are used to match the different moods of the scenes. The sound tracks also reflect the popular musical taste of time. The songs are a range of a number of American grunge. Challenging and mocking as it may be to Disney, Shrek like Snow White is a feel good film although it debunks the gentility of tradition fairy tales. Snow White begins with Once upon a time. It starts a long shot of the country and the castle, then zooms in to reveal the Queen dressed in black and purple. The audience first sees the back of the Queen to make her more mysterious. Only the crown on her head shows she is a Queen. The colour of her clothes, her pale complexion, dark eyes and very red lips show that she is an evil person. When she speaks to the mirror, her accent is slightly German and in the 1930s, the Americans did not trust the Germans. This was a deliberate way to make her seem evil and let the audience know she is an enemy towards Snow White. The mirror, whos voice seems to echo from somewhere within, hell perhaps, seems to feel sorry for Snow White when it mentions her name: Alas my fair lady. The Queen however disrespects the mirror because he/she is a slave. Her voice is loud and sharp, which makes her, appear bossy. She moves swiftly, superstitiously with long strides, which make her look dominant and overbearing. When the mirror mentions Snow White, her cat shaped eyes narrow and she hoarsely repeats it to show a malicious intent toward her. It is at this point that a dissolved shot introduces us to Snow White. Shrek on the other hand has a very different opening. The audience is shown a book with calligraphy writing, which is narrated sarcastically by Shrek himself. After six pages, a voice shouts Like its ever gonna happen, at this point, a page is torn and opening shot comes. A hut is shown and Shrek continues, What a lot of ****. Just the opening scene shows it is a comedy aimed for everyone. Like the Queen in Snow White, Shrek is shown at a low angle shot to make him look more menacing. In contrast to Snow White, Shrek is different from a traditional type of hero. He uses the page torn out of the storybook, and holds it in content. He baths in muddy water unlike any traditional hero who is portrayed as clean. He breaks wind and kills fish, one of which floats to the surface. He has a gross mannerism and the signs next to his home says: keep out, Ogre beware. Shrek is portrayed as a vicious large Ogre who is anti-social who roars and scares people with his smelly mouth. He doesnt want people around and he scares them. On the other hand however, he is shown as a private but friendly character. His privacy is a defence mechanism because he thinks people dont like him. He does it to make them think he doesnt like them either. He judges people before they get to know him just like they judge him. We can see he is not as unfriendly as earlier made to look when he has a love -hate relationship with the streetwise donkey who later became his friend. The donkey is instantly loved by the audience because of its chatter. He is not intimidated by Shreks size or the fact that he comes across as scary. He compliments Shrek by saying: I respect you Shrek, youve got that I-dont-care-what-nobody-thinks attitude and I like that. Snow White on the other hand is everything like a fairy tale princess. She is very cheerful and unaware of what happens around her. She is dressed in white, a great contrast to the Queens clothes. Her high pitched voice and wide eyes represent her innocence. Her small and delicate body highlights her youth purity. Her innocent, childlike behaviour makes people associate her with themselves. The well echos her song as she sings: my prince will come. She plays with doves, a symbol of purity, and runs away from the prince in embarrassment. She sends a dove to go and kiss him because she is too modest to do it herself. Even when the prince tries to entice her, she hides behind the curtains. In a dissolved shot, the camera zooms in to show her purity. He camera tilts upwards to reveal the Queen with an evil expression on her face. Mise-en-scene, all the animals like Snow White and play with her and it shows that she is a real princess. She is gentle, shy and modest, unlike Princess Fiona in Shrek. Princess Fiona in Shrek reacted oppositely when attacked. She hits and kicks Shrek the first time they meet. She also kills a bird at one point. Snow White is pretty too unlike Princess Fiona who turns into an Ogre. This also shows the difference between the two films. We can see the two films have different characters and certainly different heroes. While both are feel good films and are targeted for all audiences, they show different humour and reflect the different times of their production. This can be seen in the language the mirror in Snow White uses which is very out dated and humour used by the donkey in Shrek. We see that the music taste has changed as well since the production of Snow White, but also that technology has improved since then. For the production of Shrek, latest technology was used to create a lot of extras, which include life-like and realistic appearances like facial expressions, background, clothing etc. In the making of Snow White however, most of the details had to be sketched out because the technology had not advanced as much.

Whilst it seems essential that the indoor environment should be stimulating cognitively, it may be wise not to neglect the emotional aspect of it.   Again, the key worker is at the centre of providing the warmth needed in an enabling environment. Nutbrown and Page (2008) assert that they should show warm responses to the children under their care and react well to them. In essence, an enabling environment is made up of the components of suitable and stimulating indoor and outdoor areas, which the key worker is central in maintaining and facilitating. Principle 4- Children develop and learn in different ways and rates The final principle espoused by the EYFS framework is perhaps more concerned with the cognition of children, although it does refer to their social and emotional development in part. Nevertheless, Katz (1988, as cited in Carr, 2001, p.21) hypothesises that each child has a certain ‘disposition’, something which is distinct from learning; it is concerned more with how they react to certain situations and the habits they adopt and carry out on a regular basis. In a later document, Katz (1993) elaborates that dispositions in young children are normally learned from those around or the environment they are raised in or looked after, which seems to resonate with Bandura’s (1977) theory of modelling mentioned earlier in the assignment. Katz (1993) also articulates that dispositions are strengthened when they are acknowledged and efforts are made to continue them (particularly if they are good habits), which seems to emphasise the importance of treating the child as an i ndividual. It seems evident that all children learn and develop in different ways, as evidenced by the argument above. However, a conjecture could be made that there may be certain strategies which a practitioner or worker can implement which will result in children developing into sensible and mature adults. One way in which to do this is to cultivate a child’s resilience in their ability to complete a task or try a new activity.   Children may give up if they perceive the task to be beyond their capabilities and not extend themselves to complete this. This could be a natural response from the child (particularly if the task is incommensurate with their skillset) or it could be something that they have learned over time. Dweck (1975: 673) terms this as ‘learned helplessness’, where a child habitually gives up in the face of a challenging task or adversity, possibly because of a lack of response from the adult in encouraging the child to complete the task and persevere. Combining the sentiments expressed by Katz and the argument above, could be essential in helping children to progress appropriately, particularly when faced with unfamiliar situations and tasks.   Siraj- Blatchford et al. (2002) concluded in their Researching Effective Pedagogy in the Early Years (REPEY) study that shared sustained thinking was crucial in helping a child to tackle new problems and persist. This is defined as two or more individuals (with at least one adult ‘facilitator’) working together to complete a task, although the authors stress that each person should be actively contributing to the task, and that there must be a progression towards an eventual solution, even if that is reached straight away. This coincides with Vygotsky’s (1977) theory of cognitive development, which posits that a child will enhance their zone of proximal development (the difference between what they can do on their own and with help) if they are supported by a more know ledgeable other such as an adult or more capable peer.   Both arguments suggest that collaboration between adults and children is essential to further the child’s development, although again the manner in which this is done should be unique to the child by using questioning which is appropriate to the child’s level of cognitive development (Bloom et al., 1956). Conclusion The rationale which underpins the EYFS framework is the need to treat the child as a unique individual and consider their needs at length, in a cognitive, emotional, social and physical sense to facilitate optimum development in them. There are several ways to ensure this, including that the environment is stimulating and appropriate enough for the child’s needs, that they have access to a multitude of activities, that the key worker has a warm and fulfilling relationship with the child and that they encouraged to develop a growth mind set and persevere with challenging tasks with the facilitation of another adult. Arguably, if a childcare provider follows all of the actions above and adheres to the EYFS framework, then this should allow for children to progress to the desired level of development and maturity.
